Why Hospice Care?

What exactly is hospice care? Hospice or end-of-life care, is the care that individuals choose to receive when they no longer wish for aggressive treatments or there are no further treatment options. Hospice care is the focus of keeping the individual comfortable and pain free. Each individual is unique, so each case is unique. It is such a vulnerable time in an individual’s life when they decide to sign with hospice care. That is why we treat each patient with the respect and dignity that they deserve.

A group of multidisciplinary team members work together to provide comfort relating to physical, emotional and spiritual pain. The multidisciplinary team consists of registered nurses, CNAs, social workers, chaplains and a medical director. Each team member’s role is different but we come together to provide the utmost care to our patient’s and to be sure that their wishes are honored. With our medical director overseeing each plan of care for our patients, our nurses are always in contact with any medication questions or changes in condition.

Emotional and spiritual support will be provided from our social worker and our chaplain. Whether this is your first experience with hospice or you have been through it before, hospice is a tough time for everyone. Emotional and spiritual support is just as important as the physical needs from the nurses and CNAs. Whether community resources are needed, counseling or just an ear to listen, we are all here as a team to support your needs.

Our hospice agency is also available 24/7. It is a critical resource for reassurance that you and your loved ones are not alone during this time. Nurses are on call after hours and on weekends to attend to your needs or answer any questions that you may have.

We don’t know what each family is going through during this time, but we are here for support when you need it the most. Our goal is to provide each of our patients with the quality of life that they deserve. You are not alone on this journey, we are here with you every step of the way.
